Current patients receive a link for their online patient portal at their first appointment. If you have not yet logged into this portal, check your email for the link you should have received at your first appointment. If you cannot find that email with link, please contact us.
Need your medical records? Current or former patients can request their medical records anytime by downloading and submitting our Records Release Form (English or Spanish) or by completing the online Medical Release Form.

Become a Patient
All new patients must complete an Enrollment Application and then bring required documentation,
in person, on a Monday of their choice (10 am-Noon; 2 to 4 pm).

There is no cost for patient appointments. However, a $20 donation is suggested at each visit to
help towards the next patient coming through our doors.
